fLYER REQUEST FORM

Flyer request forms are available to publicize events, announcements, etc. These online flyers will be made by the PR Leadership commission.

These forms can also be found in the Student Center PR door.

daily bulletin request form

Fill out, and submit the google form by 1:30PM the day before you would like the announcement to run. It will be announced in the daily bulletin during Husky/Flex (sign in with FUSD account)

fundraiser or activity request form

Fundraiser or Activity Request Forms must be completed before any Fundraiser or Activity On-Campus or Off-Campus occurs. Completed Forms can be turned in to the Black Cubby in the Student Center.

Please file any Fundraiser or Activity Forms at least TWO WEEKS prior to the fundraiser/activity. Forms are usually passed in weekly Business Meetings. You will be contacted once your Fundraiser or Activity is approved or rejected. If you have any questions, please contact ioc@washingtonasb.com.
